these addresses and inquiries on my report are not mine. How do I get them off?

1. How do I remove incorrect home addresses from my credit report?
2. How do I remove inquiries from my credit report (those that affect my scores)?

some of those addresses are not mine, ever! and I don’t have

a clue as to where all the inquiries come from considering I have only applied for credit twice in 2 years
